Brand: Reena Gupta DDS INC d/b/a Smiley Doctor / Smileydoctor. Program: appointment reminder texts only (no promotional or marketing SMS) for San Jose and Antioch dental offices. 1. In-clinic paper / tablet consent (unchecked by default)

At check-in, the patient provides a mobile number on the registration or contact-update form. The SMS consent checkbox starts blank. Staff does not mark it for the patient. Reminder texts are sent only if the patient checks the box (or otherwise affirms consent) and a mobile number is on file.

Staff records “yes” (opted in) or “no” in the patient record. No reminder texts are sent without a yes.
